0 damage even though combat report shows a shot being hit/registered

Not sure if this is a visual bug, someone cheating or who knows what. Lately I have noticed on multiple occasions that I hit a shot, see the bloodsplats, etc. and even the combat report post round shows that I hit a shot but it shows 0 damage. It has started to happen more and more frequently and I am wondering if anyone else has had this?

I kind of had this. One time, I was fighting an enemy Viper, and she killed me. The combat report showed that I had outputted 160 damage at her, but she was still not dead.
Another time, someone killed me, but the combat report showed that I had received 3 headshots from one enemy, from a Vandal.
